Определены критерии лабораторной и инструментальной диагностики острого билиарного панкреатита, оптимальные сроки выполнения эндоскопического вмешательства. Подробно описана эндоскопическая ретроградная холангиография при различных анатомических вариантах большого сосочка двенадцатиперстной кишки, ревизии общего желчного протока, литэкстракции и литотрипсии. На основании анализа полученных результатов показана безопасность эндоскопических манипуляций и холангиографии при остром билиарном панкреатите. Высокая эффективность предложенной тактики лечения при остром билиарном панкреатите позволила сократить расход на лечение этой группы пациентов, избежать гнойнонекротических осложнений. Летальных исходов среди анализируемых больных не отмечено.

Laboratory and instrumental diagnostic criteria of acute biliary pancreatitis and optimal terms of endoscopic intervention are distinguished. In detail is described endoscopic retrograde cholangiopancreatography in different anatomical variants of papilla Vateri, examination of common bile duct lithextraction and lithotripsy. According to obtained results analysis safety of endoscopic manipulations and cholangiography in acute biliary pancreatitis is presented. High efficacy of the offered strategy of acute biliary pancreatitis management enabled to reduce expenses in this group of patients and avoid septic and necrotic complications. No one of analyzed patients died.
