Despite of the essential successes achieved now in a field of domestic and foreign oncology, the basic operation, performed at the invasive forms of the cervical carcinoma is an extended hysterectomy with ovaries' extirpation (operation Е. Wertheim). Besides a radicalism of the operation an important aspect is a quality of patients' life after performed treatment. Taking into account an annual increasing of a rate of the cervical carcinoma among women of a reproductive age, this question gains in importance. Modern line in an oncology is an introduction of an organ saving and a function sparing methods of treatment. Last years many oncogynaecologists have been saving the function of ovaries, performing their transposition from pelvis into abdomen at surgical or combined treatment of invasive forms of the cervical carcinoma. Sparing of the function of the transposed ovaries justifies that choice. Reserved ovaries don't render negative influence on 3 and 5-years results of the treatment in comparison with the standard volume. Methodically correctly performed transposition and perioperative radial therapy don't affect the function of preserved ovaries. Duration of the function of preserved gonads is individual and depends on duration of the reproductive period of a woman. A period of adaptation of the functional activity of transposed ovaries depends on a character of treatment performed. Application of the ultrasonography with Doppler scanning for the follow up examinations of transposed ovaries allows to estimate precisely the structural changes and vascularization. Maximal systolic blood velocity, a pulsating index and a resistance index in ovarian and intraovarian arteries of transposed ovaries correlate good with an activity of the ovarian function. The informativity of ultrasonography of transposed ovaries for a definition of the ovarian function came to: accuracy – 85.7 %, sensitivity – 82% and specificity – 91.2 %.
