Гастроинтестинальная стромальная опухоль поджелудочной железы: описание наблюдения и обзор литературы

Введение. Экстрагастроинтестинальные стромальные опухоли поджелудочной железы встречаются крайне редко. В мировой литературе описано только 15 случаев. Описание наблюдения. 38-летняя женщина, жалобы на постоянный дискомфорт в животе. При КТ кистозно-солидное образование головки поджелудочной железы с вовлечением в процесс воротной вены в области конфлюенса и вблизи ее бифуркации, печеночной артерии и ветвей верхней брыжеечной вены, 9 см в диаметре. Микроскопически опухоль состоит из веретеновидных клеток, разделенных миксоидной стромой. Митотическая активность 1-2 в 50 полях зрения. При иммуногистохимическом исследовании диффузная экспрессия CD117, гладкомышечного актина. Основываясь на этих данных, окончательный диагноз – первичная экстрагастроинтестинальная стромальная опухоль поджелудочной железы. Выводы. Представлен редкий случай гастроинтестинальной стромальной опухоли поджелудочной железы. Дифференциальный диагноз следует проводить с лейомиосаркомой, шванномой, плеоморфной саркомой, воспалительной миофибробластической опухолью, солитарной фиброзной опухолью, фибросаркомой и веретеновидноклеточной карциномой.

Gastrointestinal Stromal Tumor of a Pancreas: Case Report and Review

Introduction. Gastrointestinal stromal tumors of a pancreas are quite uncommon. There's described only 15 cases in the world. Case report. In 38-years old woman, suffering a constant abdominal discomfort, CT examination revealed a cystic-solid tumor of the pancreatic head nine cm in diameter. Microscopically the tumor consisted of fusiform-cells, divided by myxoid stroma. Mitosis activity was 1-2/50 in a field of view. Immunohistochemistry analysis revealed an expression of CD117 smooth muscle actin. Relying on these findings the final diagnosis was extra gastrointestinal stromal tumor of the pancreas. Conclusion. We submitted an uncommon case of gastrointestinal stromal tumor of a pancreas. Leiomyosarcoma, schwannoma, pleomorphic sarcoma, inflammatory myofibroblastic tumor, solitary fibrous tumor, fibrosarcoma and fusiformcell carcinoma are ought to be included in the differential diagnosis
