В работе определено влияние ведущих факторов риска развития рака молочной железы (РМЖ) на эффективность маммографического скрининга. В скрининге РМЖ участвовали 26 912 женщин в 2001–2010 гг. Факторы риска определяли с помощью анкетирования. В рамках скрининга РМЖ всем женщинам выполняли маммографию в прямой и косой проекциях. Для оценки рентгенологической плотности тканей молочной железы использовали шкалу рентгенологической плотности BIRADS. В результате проведенного исследования выявлено, что количество факторов риска развития РМЖ напрямую влияет на стадию заболевания. Установлено, что диагностическая эффективность скрининговой маммографии падает в группах с высокой степенью рентгенологической плотности тканей молочной железы. В работе обосновывается важность контроля групп пациенток с факторами риска РМЖ и целесообразность оценки рентгенологической плотности тканей молочной железы.

Present article evaluate the influence of leading risk factors established for breast cancer (BC) on the mammography screening efficiency. 26 912 women underwent mammography screening in 2001–2010. BC risk factors were determined by questionnaire. Mammography in mediolateral oblique and craniocaudal projections was performed in all women. BIRADS breast composition categories were used for mammographic breast density evaluation. The direct impact of BC risk factor number on the disease stage was revealed. Diagnostic effectiveness if screening mammography decreased in patient groups with high mammographic density. The importance of control of patient groups with BC risk factors and reasonability of mammographic density evaluation is described.
