The object of this study was to compare image quality assessed by myocardiumtobloodpool activity ratio between the different imaging protocols. 95 patients underwent 18FFDG PET scanning using 3 different protocols: the standard protocol, consisting of oral glucose load based on fasting glucose (38 patients), the trimetazidin protocol, consisting of pretreatment with trimetazidin to increase glucose consumption (19 patients) and the insulin clamp protocol, consisting of hyperinsulinemic euglycemic clamp (38 patients). The poor image quality was observed in 28.9% patients who underwent the standard protocol and 10.5% patients who underwent the trimetazidin protocol. No poor image quality was noted during the insulin clamp protocol. 18FFDG PET imaging in patients with obesity and noninsulinependent diabetes mellitus are problematic because of poor uptake of 18FFDG. The trimetazidin protocol yielded better image quality and higher rates of FDG uptake than did a standard protocol in patients with diabetes mellitus with out obesity. The insulin clamp protocol may be the method of choice in patients with obesity.
